What is AMH?

AMH (ANTI MULLERIAN HORMONE) is a chemical substance secreted by very early developing follicles in a female ovary, namely, antral and pre-antral follicles. So it indirectly gives us an idea as to the number of such follicles in the ovary of a female individual.

In females the lesser the number of such antral and preantral follicles (which later on develop in each menstrual cycle to become eggs) , the lower the AMH levels in the person’s blood and vice versa (PCOD). This is what we want to know through the AMH test in a female person’s blood.

  • AGE – Increasing the age of a woman will lead to consistently decreasing AMH levels.
  • PCOD – The serum AMH level is two- to three-fold higher among women with PCOD.
  • ENDOMETRIOSIS – It can cause the destruction of ovarian follicles by destroying normal ovarian tissue.
  • OVARIAN SURGERY – Ovarian cyst surgery can result in the removal of normal ovarian tissue or destruction of ovarian tissue by cautery leading to decreasing AMH.
  • CHEMOTHERAPY – Decreases AMH levels.
  • CONTRACEPTIVE PILLS – Long-term use can decrease levels up to 20%, but generally recovers to normal levels after 3-6 months of stoppage of pill intake.
  • OBESITY – For infertile women of later reproductive age (above 35 yrs) higher BMI causes AMH to decrease by up to one-third.

Age Group  –   AMH Range

  • < 26 yrs     –  3.6
  • 27-34 yrs   –  3.3
  • 35-39 yrs   –  1.6

  • What is Tubal Patency?
  • How Tubal Patency affects infertility

What is Tubal Patency?

The female body has an organ named the uterus. 2 tubular structures known as fallopian tubes are attached to it on its 2 sides. These tubes are hollow inside to let things pass through it. If these tubes get blocked then nothing passes through them.

Now that we know the above, let’s know what the most important thing that passes through these is. It’s the sperms and eggs. These meet each other inside the tubes, from the early embryo and then reach back into the uterine cavity to implant into the inner uterine wall(endometrium).

So it’s clear that if these tubes lose their patency the process of sperms and eggs meeting does not happen. So no embryo is formed. This is the reason of infertility in these individuals.

NOTE: Even if one tube is patent, infertility is avoided. Its diagnosed through procedures like HSG, SIS or LAPAROSCOPY.

Blocked tubes can be opened in some cases by hysteroscopy and laparoscopy. The success of this depends on the cause of the blockage. Previous bilateral tubal clipping/ligation done as part of the contraceptive procedure is almost impossible to revert. But if tubes are blocked due to previous mild to moderate pelvic infections, reopening them is more accessible.

In order to ensure that you get the best Tubal Patency treatment, consult the best gynaecologist in Kolkata. It is advisable to go to a clinic in your vicinity as you will not be allowed to travel long-distance right after the surgery. For example, if you live in north Kolkata, try finding a good gynaecologist in North Kolkata to perform your operation. The reproductive organs found in females are the fallopian tube that acts as a connector for the ovaries to the uterus. The primary function of the tube is to support the fertilization process (merger of egg and sperm to form the embryo) in females during the period of the menstrual cycle, every month, wherein the tubes transfer an egg from the ovary to the uterus. Know about the opening of blocked tubes.

What happens when the fallopian tubes are blocked?

If the opening of the tube is blocked the pathway for sperm to reach inside the tube for fertilization is distorted with a simultaneous deterrent for the egg other way. The reasons could possibly be scar tissue, pelvic adhesions or infections. Thereby, leading to the grave issue of infertility.

Diagnosis

There are no such bodily symptoms faced by a female when the pelvic region is affected. However, there might be history of previous vaginal or uterine infection in many cases. Processes like of X-ray called Hysterosalpingography (HSG), USG called Saline Infusion Sonography is being used by medical professionals to examine the fallopian tube obstructions. These procedures might be painful. Laparoscopy can be done under general anesthesia to find out patency of tubes in a completely painless procedure. These treatment procedures is provided by

● Hysteroscopy and tubal culation
● Through Laparoscopic surgery, less intense scar tissue is removed from around the tubes.
● But if the scar adhesions are spread in a large-scale area, then there’s no possible treatment to cure it. However, the process of healing can be gradual.
● If a part of the tube is damaged, perhaps due to ectopic pregnancy or mild infection, then the surgeon will discard the affected part.

The problem of infertility can be cured by using the above-mentioned treatments, however, patience is the key element here!

Adhesions can have a number of reasons behind their cause. It can be the result of previous surgery, infection etc. Dr Deborjyoti Pal is offering affordable infertility treatment in North Kolkata and he has been dealing with different cases of gynaecology for years. It is highly recommended to consult with a medical expert like him in order to deal with the difficulties which might be caused due to adhesions.

What is Asherman syndrome?

The adhesions inside the uterine walls might have been caused due to an injury or infection of the endometrium, which has further damaged the lining and has led to a scar in between the uterine inner walls which often end up sticking with each other. The adhesion bands inside the uterus are referred to by the term Asherman syndrome.

The most probable cause behind this is an operation/procedure named Dilatation and Curettage (D/C) done in cases of termination of pregnancy and spontaneous miscarriage. An infection of the endometrium, radiation injury, sutures inside the uterine cavity during cesarean section to control bleeding can also be a less common causes which can lead to the Asherman syndrome.

Long-term effects of this Syndrome and Cure?

Sometimes an Asherman syndrome is cured with hormonal treatment or quite often surgical treatment can be the only probable way to deal with this. There are a number of options for hysteroscopy treatment in North Kolkata with the help of which adhesion can be treated effectively. Hysteroscopy is one of the most beneficial ways to diagnose and treat cases of Asherman syndrome.

It leads to infrequent periods, stoppage of periods, infertility, miscarriage. A successful pregnancy is often possible after the complete treatment of Asherman Syndrome, but a woman needs to opt for adoption or surrogacy if the condition can’t be treated adequately.

While some women have just stretchy bands of scar tissue others might also end up having severe formation of thick adhesion bands. The type of adhesion bands can vary from person to person and that is why it needs the special assistance of an expert to be dealt with effectively. Dr Deborjyoti Pal is known for offering the best hysteroscopy treatment in Kolkata and that is why it is highly recommended to seek advice from surgeons like him.

The process of removing the uterus and ovaries also consists of a lot of side effects therefore the approval of an experienced doctor should be the first step before opting for the surgery.

Types Of Hysterectomy

There are three major Hysterectomy processes that doctors usually suggest depending on the disease condition. This treatment is recommended for women suffering from abnormal uterine bleeding that is sometimes painful and unbearable. The different types of Hysterectomy are-

Radical Hysterectomy

In this process, the uterus, cervix, upper vagina and parametrium is completely removed. The ovaries and fallopian tubes are also removed with the help of this process.

Total Hysterectomy

In this process, the uterus and cervix are removed. The removal of the ovaries is optional and the consultation of a gynaecologist is required. In a total hysterectomy, the ovaries are not much of a concern.

Subtotal Hysterectomy

In this process, only the uterus is removed and the cervix is left in situ. This method is also known as partial hysterectomy.

It is the duty of the doctor to suggest which type of Hysterectomy is important for the patient, depending on the condition. This surgery requires extra care therefore the doctor needs to be experienced. These problems require in-depth diagnosis so that no flaw is made in the treatment process.
